To rent the SRS mailing list, please complete the forms below and fax these
documents, along with a sample of your mailing(s), to the SRS National Office at 708-492-0943. Upon receipt of your mailing piece and
completed order form and agreement(s) (including payment, shipping information,
and signed mail house agreement form if applicable), the SRS will review your
application. Use of the SRS mailing list is contingent upon approval of the
mailing piece. Please allow two weeks for processing.
The SRS Mailing
List Order Form will NOT be reviewed
unless the order form and agreement(s) are complete, signed, and sample is
attached.
